Understanding Your Payment Options
Dental Insurance
We accept most major dental insurance plans and will assist you in claiming the benefits you are entitled to. As a courtesy to our patients, we will file your insurance claims, track them, and do the necessary follow-ups. Please remember that any amount not covered by your insurance is your responsibility and is due at the time of service.
Direct Payments
We accept the following forms of payment:
- Cash
- Personal checks
- Major credit cards (Visa, MasterCard)
Flexible Payment Plans
For extensive treatments, we understand that immediate payment may not be feasible for everyone. Therefore, we offer flexible payment plans that allow you to spread the cost of your treatment over several months. These plans are available upon credit approval and can significantly ease the burden of large expenses.
Financial Agreement Form
To ensure clarity and mutual understanding regarding our financial policies, we require all patients to sign a Financial Agreement Form. This agreement outlines your responsibilities regarding co-pays, deductibles, and payments for non-covered services. It includes:
- Responsibility for Payment: All charges are your responsibility, regardless of insurance coverage.
- Collection of Dues: In case of delayed payments, an interest of 1.5% per month (18% per annum) may be charged on the outstanding amount. Accounts overdue by more than 90 days will attract a higher interest rate and may involve additional collection fees.
- Missed Appointments: We charge a fee for appointments canceled with less than 24 hours notice to compensate for the reserved time.
Important Notes for Our Patients
- Health Risks and Workers’ Compensation: In the event of significant exposure during treatment (e.g., needle stick), consent for testing for communicable diseases will be obtained and followed according to state and local laws.
- Minor Patients: Payment for services for minor patients is the responsibility of the accompanying adult.
